Strange Rectangle as out of range indicator

For all actions which normally show an out of range indicator dot in the upper right corner I can see a rectangle instead. This rectangle does not appear if the button has an hotkey assigned (e.g. the lower button bar) and otherwise it appears grey (in range) or red (out of range). https://i.imgur.com/4yndYRb.png Any ideas how...
