The layout works with oUF_MovableFrames. Each frame can be scaled/moved ingame to your liking. I don't care that much on positions right now.

I'm not sure how it would look... but did you try having the portrait height being
health + power + first row of debuffs? Or do you only have the player buffs shown (usually not enough to fill a whole row)?
