Version 4 may have been the original ..
This is a link on CurseForge .. upload 2010 as v5. I assume the v5 may be what version of wow it supported.

In fact the screenshot posted on this forum is from the 2010 image upload. Seeing as the original creator has reuploaded it on wowinterface ( the link provided) to work with current wow maybe they still have screenshots from 2009 if the 2010 screenshot is still too recent for you.
