If this only happens when you die then I don't have a clue. It MAY be a conflict with another addon, but I'm not sure, as I've never had this happen.
However: If this happens when you enter combat, then there's a simple fix. At the top right corner of your map there are two small icons. Right click the farthest to the right, and you will get a menu with a few options on it. Make sure the "Hide in combat" box is not checked.
