Looks like some kind of very minimalist DK Rune tracker mod with some HP/Runic Power tracking as well. Or you using a UnitFrame mod that has a minimalist Rune Tracker.

Well its carbonites default info 1 2 window....
You can enable it if you right click the carbonite minimap icon, shows and hides it and can disable it by going into options....
