Mod for boss frames?

I'm wondering if there's a mod that can create a boss frame similar to the blizzard ones that show on certain encounters. Except it shows on all boss encounters.

Well, the boss frames are there when you want to keep tabs (be able to focus) on multiple mobs at once. I believe what you're looking for with single boss fights is the focus frame.

Both Deadly Boss Mods and Deus Vox Encounters (DXE) do boss health bars for raid bosses.
