Well I assume you are not referring to the tooltip .... so I would hazard a guess that the addon you want is Dr Damage.

The tooltip is from DrDamage.. the unit frame; could be Pitbull or ag_UF. I don't use either but I'm quite sure they can both look like that. The 3D model of yourself could be a plugin for either.

The extra info in the Tooltip is provided by Dr Damage

The Unit Frame is Discord Unit Frame

The 3d model in the Unit Frame is Mazzle's own 3d models and is apart of the Mazzle UI installation.

The little button above the Unit Frame is the action button for SmartBuff
