Could anyone tell me the name of this Unit frame addon and please give me a link to a place it can be downloaded from. I have checked the common places like cuse-gaming etc but cant find it.

Nurfed Unit Frames, or Discord Unit Frames. Looks like NUF though.

It looks like it's either ag_UnitFrames or Pitbull
most of the unit frame addons tend to look similar, and they are all highly customisable so you could make any of the listed addons look similar to the one in the screenshot.

You can't get that look with ag_UF or PitBull. It has to be either hard-coded in like that or customizable enough to get that way.

Probably a custom layout for Nurfed Unitframes (part of NurfedUI) as others mentioned. Browsing through the Nurfed Unitframes Layout forum turned up this:
