what UI? you did not post a picture or name of the UI your referring to.

nice looking UI but im not sure whos it is. might try looking in the compilations & suites and see if it's in there. or that person could have done it them self.

some addons i do see is fubar with adds, unit frames might be ag_UnitFrames,icehud,eepanels,sct,Elkano's BuffBars,cyCircled,
grid and some other stuff.

Why don't u go on wowarmory.com
the us one.
and search for "lena" the name of the shaman on the picture.
There are only 2 female draenei shamans over level 40.
Ask him.

Just also FYI if you read the chat log thats a really old pic. It may be discontinued or not work anymore. At least i think thats SimpleCombatLog down there and that isnt working since the combat log changes.

Any making your own UI is kinda your own thing. If your just starting you may want to look into a compilation and then add and remove as you see fit.

If you want to jump right in. Grab the addons your want and just move them around. Customize them skin them. If you dont know what any of this means or what to get You could either ask around here. Or once again take a look at some compilations.
