This is with all other add-ons off and along with Carbonite Nodes.

Little update, I tried to finished each quest as fast as I could and being 80 trying for loremaster didn't take long. It seems the culprit was a quest called:

Boulderslide Ravine

Happened to me at the Protetorate Watch Post too. Wondering if the trigger was simply grabbing a bunch of quests at once.

We'll see if 4.0004 fixes it
