Looks like any number of info bar addons. It could be one of numerous varieties/styles of info bars and/or plugins for those bars.

LDB style are one type, Fubar is a completely other type, Titan I believe is 100% compatible with LDB. Don't quote me on that though

I will be quite honest it doesnt look like an info bar plugin such as fubar/titan panel/any data broker bar mod. I would venture a guess that it is just a graphic/viewport mod as when I zoom all the way in on the picture (ctrl + mouse wheel for firefox users) the text above the minimap isn't centered on the bar which leads me to believe it is most likely a mod like kgpanels combined with sldatatext or whatever it is called or litepanels with litestats. I have used litestats and litepanels for the same affect in some of my previous uis I have made. At any rate it doesn't look hard to do the easiest way to get the same affect if you don't want to fiddle around with any lua crap would be to nab yourself kgpanels and sldatatext (or a data text mod like it) and whip it out real quick
