Barring Entry Quest bad crash
This quest: http://www.wowhead.com/quest=30634
blows up Carbonite. It happens just when you leave the "safety" of the invisibility ring, and click to setup the bomb. Its like a new thing that Blizzard is doing, and Carbonite just gets really silly, and of course you die.
